IRS Form 14039 is an identity theft affidavit that you send the IRS if your personal information has been stolen. After the IRS processes your identity theft affidavit, the IRS will place an identity theft indicator on your tax account to show that your personal information was stolen and to help prevent future tax refund fraud.
The IRS will also send you an Identity Protection Personal Identification Number (IP PIN) in December or January. The IP PIN is extra confirmation of your identity when you file your tax return. It helps prevent another individual from filing a return using your Social Security Number. The IRS will issue you a new IP PIN each year for as long as identity theft is an issue with your IRS account.
